1.Requests for disclosures, etc.

Please post requests for disclosure, etc. to the address below, after attaching the necessary documents to the prescribed disclosure request form. When posting the request form, please write "Personal Information Disclosures, Etc. Request Enclosed" in red ink on the envelope.

2. Documents required for requests for disclosures, etc. (format)

When requesting disclosures, etc., download the prescribed disclosure request form (A), complete the designated items, enclose an ID verification document (B) and mail the documents to the address indicated in (1) Requests for disclosure.

3. Requests for disclosures, etc. by a proxy

When any legal proxy of a minor or an adult ward makes a request for disclosures, etc., or when any principal delegates a proxy to request disclosures, etc., please include documents stipulated in (A) and (B) below in addition to the form mentioned in (2) Documents required for requests for disclosure (format).

A. In case of a legal proxy
  • Documents verifying authority as a legal proxy (copy of a family register or insurance card listing dependent family member(s) if the legal proxy has parental custody are acceptable) 1 copy
  • Documents verifying the identity of the legal proxy (legal proxy’s driver’s license, passport (including an address) or health insurance card (blanking out the code, number, etc. of the insured person, etc. in advance. Also applies below.) 1 copy
B. In case of a proxy designated by the principal
  • Power of attorney 1 copy
  • Documents verifying the identity of the proxy (proxy’s driver’s license, passport (including an address) or health insurance card (blanking out the code, number, etc. of the insured person, etc. in advance. Also applies below.) 1 copy

4. Fees and payment method

A fee of ¥1,500 applies for each request for disclosure or notification of purpose of use. Please enclose a fixed-amount postal money order for ¥1,500 (issued by post offices) together with other documents required for the application. No fee is required for requests for correction or suspension of use.

  • * Applicants must bear the cost of postage for sending requests to Mitsui Fudosan and the fixed-amount postal money order.
  • * In cases where the fee is insufficient or not included, Mitsui Fudosan will notify the applicant to that effect. If payment is not made within a prescribed period of time, the Company shall deem that no request for disclosure has been made.

5. Method for responding to requests for disclosures, etc.

Responses will be mailed to the notification address entered in the prescribed disclosures, etc. request form.

6. Purpose of use of personal information acquired in relation to requests for disclosures, etc.

Personal information obtained in response to a request for disclosures, etc. shall be handled only to the extent necessary to process the request.

7. Non-disclosure of retained personal information

In the cases indicated below, Mitsui Fudosan cannot comply with requests for disclosure of personal information. The Company will retain the prescribed fee even in cases where it is unable to disclose requested personal information.

  • When the principal's identity cannot be confirmed, such as when addresses on request forms and identity confirmation documents do not match
  • When the authority of a legal proxy cannot be verified with respect to a request made by a proxy
  • When the subject of a request for disclosure does not fall under retained personal information
  • When there is a risk of harm to the life, body, or property of the person to whom the personal information refers, or to third parties
  • When there is a risk of considerable obstruction to the appropriate operations of Mitsui Fudosan’s business
  • When disclosure would constitute a violation of a law or regulation
